For proper treatment,\u00a0it\u00a0is\u00a0necessary\u00a0that\u00a0I, as\u00a0your\u00a0treating\u00a0therapist,\u00a0build\u00a0up a file.\u00a0This\u00a0is\u00a0also\u00a0a\u00a0legal\u00a0obligation\u00a0imposed\u00a0by\u00a0the\u00a0WGBO.\u00a0Your\u00a0file\u00a0contains\u00a0notes\u00a0on\u00a0your\u00a0state of health\u00a0and\u00a0data on\u00a0the\u00a0treatments\u00a0carried\u00a0out.<\/span> The file also contains information that is necessary for your treatment and that I, after receiving your explicit permission, have requested from another care provider, such as your GP.<\/span>\u00a0<\/span><\/p>\n

We\u00a0 do our best to guarantee your privacy. This means, among other things, that we:<\/span>\u00a0<\/span>\u00a0<\/span><\/p>\n

  • ensure that unauthorised persons cannot access your data.<\/span><\/li>\n<\/ul>\n

    As\u00a0your\u00a0treating\u00a0therapist, I have\u00a0sole\u00a0access\u00a0to\u00a0the\u00a0data in\u00a0your\u00a0file.<\/span>\u00a0<\/span>I have a legal obligation of confidentiality (professional secrecy).<\/span>\u00a0<\/span>The\u00a0 data from your file can also be used for the following purposes:<\/span>\u00a0<\/span>\u00a0<\/span><\/p>\n

    • To inform other health care providers, for example when the therapy is completed or in the case of a referral to another practitioner. This will only happen with your explicit permission.<\/span>\u00a0<\/span><\/li>\n
    • For\u00a0use\u00a0as\u00a0an\u00a0observer\u00a0during\u00a0my\u00a0absence.<\/span>\u00a0<\/span><\/li>\n
    • For\u00a0anonymised\u00a0use\u00a0during\u00a0peer review.<\/span>\u00a0<\/span><\/li>\n
    • A small part of the data from your file will be used for financial administration so that I can prepare an invoice.<\/span>\u00a0<\/span><\/li>\n<\/ul>\n

      If\u00a0I want\u00a0to\u00a0use\u00a0your\u00a0data\u00a0for\u00a0any\u00a0other\u00a0reason, I\u00a0will\u00a0first\u00a0inform\u00a0you\u00a0and\u00a0ask\u00a0your\u00a0explicit\u00a0permission.<\/span>\u00a0<\/span>The data in the client file will be kept for 20 years, as required by the law on the treatment agreement. You have the right to inspect the file, to change or to delete items at all times.<\/span>\u00a0<\/span>\u00a0<\/span>PRIVACY ON THE CARE INVOICE<\/span> \u00a0<\/span>The are invoice you receive will contain the information required by the health insurance company, so that you can claim this invoice from your health insurance company.<\/span>\u00a0<\/span>\u00a0<\/span><\/p>\n
